The position is for an independent contractor and the service area is Southern California particularly Los Angeles County, Orange County,Ventura County,San Bernardino County,Riverside County,Santa Clarita County.
Second language knowledge is a plus, we are particularly looking for Chinese, Farsi, Spanish, Tagalog, Russian, Armenian speaking Physical Therapists.

Duties:

Educates the family members or the caregivers about the challenges that they or the patient might face within the treatment process.
Ensure equipment is prepared and safe for the patient treatment.
Reviews patient’s medical histories and diagnoses patients by observing their movements and listening to their complaints.
Documents and reports patient’s progress to the physicians and family members.
Interpret findings from examination, diagnostic studies/medical test, select appropriate clinical test and measures and integrate findings into the physical therapy plan of care for the full range of patient recovery program.
Establishes care and treatment plans for patients using different methods and methodologies. Including therapeutic exercises, stretching, hands on therapy to help them gain a pain free state of health and achieve the desired mobility level.
Evaluates and offers treatments to patients who are recovering from injury, accident, disease or surgery in a home care basis.

Qualifications:

Home Health Care Experience (Preferred)
PT must operate under one’s own business entity such as an LLC, S-Corp, or Professional corporation
Home health Care Experience (Preferred).
Valid Driver’s License and State required Auto Insurance.
BLS Certification
Valid California Physical Therapy License
New graduates of approved programs are welcome to apply.
Degree in Physical Therapy from an approved program pursuant to Medicare guidelines

Duties:

Provides education and training to patients and caregivers. Performs other job-related duties as assigned.
Documents patient treatments as well as medical and progress report.
Provides daily and timely patient follow up documentation of interventions and services provided in the care plan.
Observes for changes in patient responses and reports changes to doctors and physical therapists in charge.
Observes and follows treatment plan as identified by physical therapists or physicians.
Instructs and guides patients through functional activities, such as bed mobility, transfers and balance activities.
Performs physical therapy treatments, including the use of manual techniques and common exercise techniques.

Qualifications:

PTA must operate under one’s own business entity such as an LLC, S-Corp, or Professional corporation.
Home health Care Experience (Preferred).
Valid Driver’s License and State required Auto Insurance.
BLS Certification.
Valid California Physical Therapy Assistant License.

Graduate of an accredited Physical Therapy Assistant approved program by American Physical Therapy Association, Inc.

Duties:

Demonstrates progress toward meeting the criteria for professional practice.
Provides accurate, timely documentation of occupational therapy patient management.
Discharges patient from occupational therapy when the patient has received maximum benefit from occupational therapy.
Regularly assesses clinical signs and symptoms to determine effectiveness of treatment, progress of patient towards goals, the need for modifying goals and treatment.
Provides advice and education to patient and family.
Coordinates the treatment program with other health care personnel as appropriate.
Implements the treatment program through direct treatment of the patient.
Initiates discharge planning for occupational therapy.
Establishes treatment goals and plans treatment to achieve established goals.
Evaluates patients and interprets evaluation findings to determine the nature and extent of dysfunction.

Qualifications:

Home Health Care Experience (Preferred).
Occupational Therapist must have one’s own business entity such as an LLC, S-Corp, or Professional corporation.
Valid California Occupational Therapist License.
BLS certification
Licensed/Registered by the Occupational Therapist Board of California

Master’s Degree in Occupational Therapy.

Duties:

Updates patients records for the Occupational Therapist.
Instructs patients and patients' families in the use and care of equipment and devices such as exercisers, mobile arm supports, splints, adaptive utensils, self-care aids and tools used in the treatment plan.
Advances the patient’s motor skills through progressively more laborious activities.
Implements treatment procedures to stimulate muscle activity to develop or improve movement, including exercise, activities of daily living, arts and crafts activities, social activities etc.
Encourages patients with their recovery process.
Assists occupational therapists with patient evaluation by performing standardized evaluation procedures such as range of motion measurement, grasp and pinch measurement, and homemaking checkout.
Facilitates patient participation in therapy activities by discussing with patients and the therapist their goals and treatment objectives.
Ensures and monitors patient’s progress so that they are following the established treatment plan.

Qualifications:

Home health Care experience (Preferred).
Valid California Certified Occupational Therapy Assistant License
BLS certification
Certified Occupational Therapy Assistant must have one’s own business entity such as an LLC, S-Corp, or Professional corporation

Identify patient/client needs and develops quality programs to meet those needs.
Develops treatment plan to have the maximum feasible therapeutic impact on the patient.
Provides consultations, when appropriate, to patients, family members, caregivers etc.
Increases the patient's family's/caregiver's understanding of the patient's therapy options, encourages family participation in the decision-making process.
Educates and teaches patients for speech related drills and guides them how to make sounds while improving their speech and voice as well as focusing on fluency.
Creates and carries out customized treatment plans addressing the specific functional needs and requirements.
Evaluates levels of swallowing as well as speech and language disorders.
Identify, assess, and provide treatment for communication, cognition, and swallowing functions and their disorders

Qualifications:

Home Health Care Experience (Preferred)
SLP must have their own business entity such as an LLC, S-Corp, or Professional corporation
Minimum 1-year experience in providing Speech and Language Services.
Valid California Speech Language Pathologist License.
BLS certification
